Is a Lower Body Lift Worth It?

Deciding whether a lower body lift is worth it: who it helps, the scars and recovery it asks of you, how long results last, and the questions to ask first.

Key takeaways

  • It removes a “belt” of tissue from around the lower trunk, eliminating lower back rolls and giving some elevation to the outer thighs, buttocks and mons pubis.
  • ASPS says body contouring is ideally done once weight has held steady for two to six months, often 12 to 18 months after bariatric surgery.
  • The scar is permanent. ASPS puts it plainly: visible scars are something you must accept.
  • Recovery runs four to six weeks on average, with swelling above the scar for two to four months.
  • Final contour usually takes 12 to 24 months, and holds only if your weight stays stable.
  • Complication rates differ by study and by how each one counts: 7.3% major complications, and about 16% of any severity, in a single-surgeon series of 110 patients with a body mass index under 30; 35% of any severity in a 40-patient outpatient series; and 78% of any severity (22% major) in a retrospective series of 100 patients after massive weight loss.

If you’ve lost a lot of weight and you’re left with loose skin around your waistline, a lower body lift is probably the operation you keep running into. Whether it’s worth it depends on what you want fixed and what you’ll trade for it: weeks of recovery and a permanent scar around your body.

Your surgeon’s instructions outrank any general schedule, including this one.

What is a lower body lift

In a surgeon’s terms, ASPS’s 2017 practice parameter files a lower body lift under circumferential lipectomy, or belt lipectomy: abdominoplasty or panniculectomy plus removal of excess skin and fat from the lateral thighs and buttock.

It removes a “belt” of tissue from around the lower trunk, eliminating lower back rolls and giving some elevation to the outer thighs, buttocks and mons pubis. ASPS’s procedure steps say an incision around the body removes an apron of excess skin and fat, and repositions and tightens tissues.

Who benefits, and who isn’t ready

Candidacy is a surgeon’s call after an exam, so read these as general criteria. ASPS’s candidate criteria name significant soft tissue looseness, good health without conditions that impair healing, realistic goals, nonsmoking status, and a commitment to nutrition and fitness.

Timing is usually the reason a surgeon says not yet. A lower body lift after weight loss is ideally done once your weight has held steady for two to six months, which after bariatric surgery often means 12 to 18 months out, or a body mass index of 25 to 30.

  • If you’re still losing weight, it may be best to wait. The guideline says procedures are sometimes staged, starting with a functional panniculectomy in Farmington Hills to improve comfort and ease exercise.
  • If you still smoke. It says smokers should be instructed to stop, because smoking can impair postoperative wound healing, and that some surgeons won’t operate on smokers. One such series recommended six to eight weeks off.
  • Expect bloodwork. The guideline names protein, calcium, vitamin D, B12 and iron as the most common long-term deficiencies after bariatric surgery, and also screens depression, diabetes, reflux and hernias.

Lower body lift scars

Lower body lift scars are long and permanent. ASPS says the procedure requires extensive incisions, and that one common technique uses incisions similar to a bikini pattern that travels around the body.

Ask your surgeon where the line will fall. In a review of 40 outpatient near-circumferential lifts, the surgeon wrote that the scar is placed to avoid any encroachment on the buttock, and that a scar across the posterior midline may be avoided.

  • It stays visible. ASPS is blunt: visible scars are something you must accept to achieve the contour. The line’s appearance changes; the scar does not go away.
  • First months: a clinical review of cutaneous scarring reports that the redness of a scar fades after seven months.
  • Lower body lift scars after a year: the same review says maturation is complete on average after one year and that normal scars should be flat and pale by then. MedlinePlus adds that scars grow less noticeable as they age.
  • When it doesn’t settle: unfavorable or hypertrophic scarring is on both ASPS risk lists, and sutures can surface through the skin. Scar deformity, including dog ears, was recorded in 13% of that 40-patient series, the highest single entry in its complication table.

Lower body lift recovery

Lower body lift recovery usually starts in a hospital. The guideline says extensive circumferential procedures are usually done in a hospital under general anesthesia, with a stay of one to four days, and that careful outpatient monitoring in the early period may be an acceptable alternative where a surgeon is set up for it.

It also describes a urinary catheter to monitor fluid status in more extensive cases, and an epidural some surgeons use for the first 24 to 48 hours, for pain control and to get you moving early. You may have one or several drains.

Extensive circumferential procedures require on average a four- to six-week recovery, and swelling above the scar persists for two to four months.

  • Drain care is a daily job. ASPS’s recovery guidance says thin tubes may be placed temporarily under the skin to drain excess blood. Line up help at home.
  • Appointments are frequent at first: weekly until the drains are out and seromas have resolved, then monthly for three months, then at six months and a year. Arrange rides; driving is your surgeon’s call.
  • Protect the incisions, which ASPS says shouldn’t meet excessive force, motion, swelling or abrasion while they heal.

How long lower body lift results last

Lower body lift results show up early and settle slowly. ASPS says they’re visible almost immediately and may take two years or more to fully develop; the guideline puts final contour maturation at 12 to 24 months.

Durability depends on your weight. ASPS says the overall results are long lasting, provided that you maintain a stable weight and general fitness, and that most of your initial improvement should remain as your body ages.

ASPS states there is no guarantee, and recurrent looseness of skin is a listed risk.

The risks, plainly

ASPS’s risk list reflects a long operation on a lot of tissue.

  • Anesthesia risks; bleeding (hematoma); fluid accumulation; infection
  • Deep vein thrombosis, cardiac and pulmonary complications
  • Poor wound healing; skin loss; fat necrosis; asymmetries
  • Numbness or other changes in skin sensation, which the guideline says may be temporary or permanent; pain, which may persist
  • Persistent swelling in the legs; skin discoloration; recurrent looseness of skin
  • Unfavorable scarring; sutures that surface and need removal; possible need for revisionary surgery

Published rates vary widely, and the studies define their terms differently. A 2016 retrospective series of 100 patients after massive weight loss, 87 of them after bariatric surgery, reported complications in 78%, 22% of them major, with wound dehiscence the most common.

A 2026 single-surgeon series of 110 patients with a body mass index under 30 reported major complications in 7.3%, with complications of any severity in about 16%. The 40-patient outpatient series, also one surgeon reporting his own results, reported complications of any kind in 35%, without separating major from minor. That 100-patient series also tied complications to body mass index before surgery.

There are two tiers of warning signs. ASPS says that if you experience shortness of breath, chest pains or unusual heartbeats, seek medical attention immediately. Call 911.

The second tier is a call to your surgeon’s office. MedlinePlus says to contact your provider right away for redness, increased pain, or yellow or green pus or excessive clear fluid around the wound; black edges; bleeding that won’t stop after 10 minutes of direct pressure; a fever of 100°F (37.7°C) or higher for over four hours; pain that persists despite pain medicine; or a wound that has come open.

Lower body lift vs tummy tuck

The lower body lift vs tummy tuck choice comes down to where the loose skin sits, and which one fits is a surgeon’s call after an exam. A tummy tuck treats the abdomen. Rolls across the lower back and loose outer thighs are beyond its reach.

What it won’t doReplace weight loss; correct stretch marks, except any sitting on the skin removedNot a weight-loss operation: a 2023 retrospective review found body contouring was not itself associated with long-term weight loss

ASPS warns a tummy tuck’s outcome can be greatly diminished by significant weight fluctuations, and the body lift carries the same condition. The post on the best body contouring procedure in Farmington Hills lines the options up.

Lower body lift regrets

The lower body lift regrets people describe cluster around a few things the sources below each speak to. Each one is something to raise at a consultation.

  • Expecting it to move the scale. In that 2023 study of bariatric-surgery patients, body contouring was not itself associated with long-term weight loss.
  • Underestimating the scar. Ask the surgeon to draw the planned incision on you.
  • Operating too early. ASPS puts the stable-weight window at two to six months, and in that 100-patient series complications rose with body mass index before surgery.
  • Smoking through it. The guideline says smoking can impair postoperative wound healing, and that series recommended six to eight weeks off before surgery.
  • Not budgeting for a second stage. ASPS says another surgery may be necessary, and 8% of that 40-patient series returned for secondary thigh lifts.

Questions for your consultation

  1. Is a full circumferential lift what you’d recommend, or would a smaller procedure do more?
  2. Would you stage this, and if so, what comes first?
  3. Can you draw the incision on me, and will the scar cross my back?
  4. How stable does my weight need to be before you’d operate?
  5. Will I stay in the hospital, and for how long?
  6. When do the stitches and the drains come out, and when can I bathe?
  7. When can I drive, exercise and work?
  8. If I need a revision, how’s that handled?
  9. Which symptoms should I call about, and which mean the emergency room?

Frequently asked questions

Is a lower body lift worth it?

Only you can weigh that. The reported benefit appears to be quality of life rather than weight loss, against a permanent scar, four to six weeks of recovery and a real complication rate.

How bad are lower body lift scars after a year?

A clinical review of cutaneous scarring reports that maturation is complete on average after one year, and that a normal scar should be flat and pale by then. Those figures are for scars in general, not for this scar specifically, and a long scar around the trunk sits under more tension than most. Redness fades after about seven months. The scar itself is permanent.

How long is lower body lift recovery?

ASPS puts it at four to six weeks on average, after a hospital stay of one to four days. Swelling above the scar lasts two to four months.

How long after weight loss should I wait for a lower body lift?

ASPS says body contouring is ideally performed after a stable weight has been held for two to six months, which after bariatric surgery often means 12 to 18 months out.
